Thank you for taking the time to fill out the registration forms prior to your appointment. The format of our registration is a print-and-fill process. Bring the completed forms with you to your scheduled appointment along with 1. picture identification, 2. medical insurance card, 3. vision insurance card (if you have one), and 4. a list of your medications and drug allergies (if applicable).
